Below you can find two attachments which you can click to download. These provide an example of a Lead Practitioner Appraisal template, a simple form for appraisals which can be added to throughout the academic year (created by St John Plessington Catholic College). This includes:
- Lead Practitioner Appraisal Guide - This includes screenshots of how this TLH Holder Appraisal could look when set up, and goes through the Outcome Sets, Fields and Focus Areas which you would need to add to your setup in order to replicate this.
- Lead Practitioner Appraisal PDF Record - This provides an example of how the record will look once the set up is complete, if following the example provided.
